Sample Questionnaire

The “My Household” section of the questionnaire asks the following questions:

• How many children do you have under the age of 18?
• How many people, not including yourself, live in your household?
• Do you own or rent your current residence?
• Do you have pets?
• Who in your household is responsible for making the majority of the decisions about day-to-day purchases?
• Who in your household is responsible for making the majority of the decisions about large purchases?
• What is your total annual household income, before taxes?

The “About Me” and “My Education” sections ask you to describe your

• gender,
• marital status,
• Hispanic heritage,
• race,
• sexual orientation,
• highest level of education,
• and whether you are currently a student.

The last two sections, “My Job/Career” and “My Politics,” ask you to explain whether you receive any income through self-employment, your current employment status and industry, your job title and occupation, and your primary place of work. It also asks what political party you are registered with, and what your political views are.
